Primary Care

If you or your child have Medicaid, we can help you in many ways. Some of the things Alabama Care Network Southeast can do are:

Need a doctor? We can set you up with a primary care doctor.

Need a ride? We can help you get a ride to doctors’ appointments.

Don’t understand your or your child’s sickness? We can explain your health conditions to you and the best ways you can take care of yourself.

Confused by your medicines? We can check your medicines to make sure you are taking the right drugs and know when and how to use them.

In the hospital? We can visit you in the hospital and help you know what to do when you get home. We can also make sure you meet with your doctor after your hospital stay. We will even visit you in your home after a hospital stay to make sure you’re ok.

Don’t know what to ask your doctor? We can meet you at your doctor’s office to help answer your questions about your or your child’s health.

Don’t forget your mental health. We can also help you get an appointment with a mental health provider or at a community mental health center if you need one.

You have the right to see any Alabama Medicaid primary care doctor no matter where the doctor is located. You can only get care coordination services in the county where you live.

Maternity

Congratulations, Mom! We want a happy, healthy pregnancy and delivery for you and your baby. We are sure you have lots of questions—and we can help!

Do you need Medicaid? We can help you sign up for Medicaid during your pregnancy.

Who will deliver your baby? We can help you find the doctor or midwife of your choice to deliver your baby.

What care do you need while you’re pregnant? We can help teach you what you can do to help you and your baby stay healthy during your pregnancy.

Need a pediatrician? Baby will need a doctor, too. We can help you choose a doctor to care for your baby after he or she is born.

Having complications with your pregnancy? We can get you to the right doctors if you have complications or if you are at high risk for complications.

What do you do once baby is born? We can visit you in the hospital after your baby is born and before you go home to help you know what to do next.

Need other help? You may have other needs, too. Maybe you need housing, rides, or WIC. We can get you help with these needs in your community.
